Lifeworks NW EASA: Registered Nurse Beaverton/Milwaukie

DESCRIPTION

LifeWorks NW’s (EASA) Early Assessment and Support Alliance provides individual/family support, education, advocacy, and treatment to persons aged 12 to 30 who are experiencing emerging symptoms of psychosis. The program also has a robust family component, including multi-family groups. EASA serves individuals who are publicly or privately insured or who have no insurance.

The Registered Nurse (RN) will serve as a liaison between Mental Health Providers and Primary Care Providers by providing ongoing nursing assessments and coordination of care with other professionals for adults with severe and persistent mental illness who elect to participate in the project. The RN may also function as a Qualified Mental Health Professional (QMHP). Provision of care coordination is primarily based in a clinical setting but may include attending physician appointments with clients or meeting with clients in their homes or at community based sites.

The Mental Health Therapist screens, assesses, diagnoses, and treats transition age youth ages 12-30 experiencing their first onset of psychosis and schizophrenia spectrum illness in its initial 12 months.  Provides psycho education, appropriate treatments, and/or consultation with families, care providers, and community partners regarding treatment options.  Works in community-based, home-based and office-based settings.  Also offers community education to reduce the stigma of mental illness and raise awareness about psychosis.

EASA was created to address the needs of young clients who are experiencing their first episode of psychosis. The program’s purpose is to provide intensive supportive services, reduce symptoms, and mitigate the impact of psychosis on the individual, family, and society. This role will provide professional evaluation, treatment, referral, and monitoring services.
